My current estimation is that this issue is caused by either mod or console use. As i may or may not have raged out and killed Ada, this will initially be focused on fixing Dogmeat for now. This bug caused me to be unable to communicate with my 'Companions' (Dogmeat and Ada are the only know ones with the issue as i haven't 'met up' with my other Companions yet to fully verify this). I recently ran into what i can only assume is a bug.
